Renewal/Replacement of Passport:

Applicants must submit the following documents:
  • A completed Passport Application form (download here)
  • Two (2) identical non-digitized color photographs, one of which should be certified by the Notary Public or consular officer who signs Section G of the form (CRITICAL: PLEASE SEE PHOTO GUIDELINES HERE – U.S. passport photos issued by many chain stores will not suffice. Also note that photos should not be cut smaller than the standard approximately 2″ x 2″ passport size).
  • Birth Certificate (current color format)
  • The Expired Passport, unless lost or stolen
  • Lost Passport Report, if passport has been lost or stolen (download here)
  • Applicants who were last issued passports as a minor or whose passport has been lost or stolen must submit a certified copy of a valid government-issued photo identification
  • Marriage Certificate, where applicable
  • Death Certificate, where applicable
  • Adoption Register, where applicable
  • Divorce Judgment, where applicable
  • Money order or certified bank check payable to “Embassy of Jamaica”
No photocopies of required certificates should be sent. All certificates will be returned to the applicant.

Fees (as of 1/1/14) are as follows:
New/Renewal – Adults – US$110.00
New/Renewal – Minors – US$80.00
Lost/Stolen/Damaged – Adults – US$190.00
Lost/Stolen/Damaged – Minors – US$130.00


Applications for renewal of a passport must be accompanied by the most recently issued expired passport. Applications submitted with previously cancelled passports will not be processed as renewals.
